What is surname,is it necessary to have surname,what are the problems one will face if they don't have surname?

Some persons in South India have only names for example Nagaraja Siddappa, Nagaraja is his name and siddappa is his father name, but don't have surname like Patel, Sharma etc. Is it necessary to have surname, what are the problems one will face if they don't surname.
